When Weekly Therapy Isn’t Enough: Understanding Mental Health IOP

Sometimes weekly therapy is enough support. Sometimes life starts to feel harder to manage between appointments.

Anxiety may be interfering with work, school, relationships, or everyday routines. Depression may make it difficult to function the way you normally do. Trauma symptoms can begin taking up more space in your life. You may be getting through the week, but feel like you're spending most of your energy simply trying to keep up.

When you need more support than a weekly therapy appointment can provide, an Intensive Outpatient Program (IOP) may be an option.

What Is a Mental Health IOP?

A Mental Health Intensive Outpatient Program provides structured mental health treatment several times throughout the week while allowing you to continue living at home and remain connected to your everyday life.

IOP falls between traditional outpatient therapy and more intensive levels of treatment. Mental health IOPs are generally designed for people who can safely spend time outside of treatment but need more significant support than weekly outpatient care provides.

Rather than waiting an entire week between therapy appointments, IOP creates more consistent opportunities to practice skills, receive support, understand patterns, and work toward greater stability.

When Might It Be Time to Consider IOP?

There isn't one moment when someone suddenly becomes “sick enough” for additional support.

You might consider talking with a provider about a higher level of care if:

  • Weekly therapy doesn't feel like enough support anymore.

  • Anxiety, depression, trauma, or other symptoms are increasingly interfering with daily life.

  • You're struggling to maintain routines, relationships, work, or school.

  • Symptoms have continued or worsened despite outpatient treatment.

  • You're stepping down from a higher level of mental health care and need continued structure.

You also don't need to determine the appropriate level of care on your own. An assessment can help you and a treatment team understand what kind of support makes sense for what you're experiencing.

What Happens in Mental Health IOP?

Mental health IOP is about more than attending additional therapy appointments.

At The Lotus Collaborative, our primary Mental Health IOP supports adults 18+ experiencing anxiety, depression, trauma, PTSD, and other mental health concerns.

Each treatment day includes a range of groups designed to support healing through connection, education, self-expression, and skill-building. Treatment incorporates approaches including CBT, DBT, ACT, IFS, mindfulness, therapeutic art, and experiential activities.

The goal is not simply to learn skills in a treatment room. It's to begin practicing them in the context of your actual life.

How Does IOP Fit Into Everyday Life?

Because IOP is an outpatient level of care, clients continue living at home and have time outside of programming to navigate everyday life and responsibilities. However, IOP does require a meaningful time commitment, and depending on an individual’s treatment schedule, work, school, or other responsibilities may need to be adjusted.

For some people, virtual or hybrid treatment can make accessing this level of support more manageable. At The Lotus Collaborative, our Mental Health IOP is available in person in San Francisco, virtually throughout California, and in hybrid formats.

Our Mental Health IOP serves adults 18+, with in-person care in San Francisco and Santa Cruz, along with virtual options throughout California.

Lotus is in-network with Aetna, Anthem Blue Cross, Cigna, Kaiser Permanente, and UnitedHealthcare, and we also work with Medi-Cal referrals. For other insurance plans, our admissions team can explore whether a Single Case Agreement may be available.
